The copyright office has put into place a new system where they ask for a list of the file names and image titles.
This is fine if you are only copyrighting a few images at a time. However, I tend to register every three months or so and end up with hundreds of images to copyright (and registering has paid off in more than a few cases, so it is worth it!). It gets a bit unwieldy to have to copy and paste each indivdual image title and name.
Is there a script that will output a cvs or text file that includes the file name and title, without the full image path?
If not, how do I go about building one?

You can use right click on a selected thumb and then choose Metadata -> Export Metadata to CSV file
There, select the applicable copyright field(s) to include in the export.
By default PSU will include the full file name. But you only need the file name without path.
In that case:
1. Define a custom macro field with the file name only like this (in the Info panel): 2. Once the custom field is created, now do the metadata export and also select the filename custom field.

Sweet moves! Yes it does.
I just tested it by selecting a bunch of thumbnails and then doing the Metadata>Export to CSV.
It gathered up all the info needed for the copyright registration form.
