As far as I know, one vocabulary text file to support two or more custom fields is not possible.

Where the custom fields are actually dropdown fields only containing those values?

If after following Hert's advice, you still have images with no keywords, you might have labels that are configured not to create keywords which could result in having images with labels assigned but still no keywords. I'm not sure how you could figure out which ones they are. You might be able to ...

An image without keywords: added a Keyword under Details - Descriptions - Keywords. Visible in Details - Descriptions - Keywords. Also visible in the Assign panel as being applied. If you click on the image, go to the Assign panel, is drainage shown underlined in green?

Under the Sync Settings in Preferences, do you have Automatically write out Catalog changes to ... enabled and what is your Write - Keyword processing setting?

Is this specific label / keyword hierarchy been around for awhile, where you may have change settings within it over time or is it brand new?

One source: other programs that write keywords that one uses, or had used, and depending on the Read settings one is using in Photo Supreme.
