As stated by RobiWan (above) your PSU keyword settings could be playing a role.
I don't use ON1 or Photo Mechanic.

Each time I open PSu, more of my Categories are appearing as ADDITIONAL categories under Miscellaneous.
Before you notice this have you imported or verified?

I am not familiar with ON1, but with Lightroom, if the photos are already in Lightroom and I change metadata in PSU, I need to use Synchronize in Lightroom. And if I change metadata in Lightroom (I know, not recommended), I need to use Verify in PSU.

What I have done in a similar situation is to place one photo in the folder I want to appear in Photo Supreme. I import the photo. The photo and folder appear in Photo Supreme. Then I drag and drop photos, within Photo Supreme, into the folder.

Excellent tests.
If the programs are writing to different sidecar files (JPG, XMP), there won't be any communication between them. If you are writing directly to the RAW file, that may also cause issues.

After adding some metadata in Photo Supreme to one of the older files, go to the file in the file system (you can do this by right clicking on the file ... Locate in ...).
Do you see:
photo.RAW (where RAW will be your camera's raw extension)
photo.JPG
photo.XMP

In your workflow, Photo Supreme is writing to the RAW, JPG, or XMP?
I haven't shot RAW+JPG in a very long time, so I am not sure what is possible, in regards to writing, with Photo Supreme.
